The last three years produced more controlled data on orthobiologics than the decade before them. That is genuinely good news — and it has also narrowed the list of claims a clinic can responsibly make.
In 2026 the strongest signals remain in knee osteoarthritis and certain tendinopathies, where multiple trials show meaningful symptom improvement in appropriately selected patients over 6 to 12 months. The weakest signals remain in conditions where the underlying structure is already mechanically failed; no injection restores a torn structure to intact.
What has changed most is preparation standardization. Reporting frameworks now let clinicians compare platelet dose, leukocyte content, and activation method between studies. When you read a claim about 'PRP', ask which PRP — the answer changes the expected result.
Our practical stance: biologics are one tool inside a plan that also includes load management, rehabilitation, metabolic health, and sleep. Patients who treat the injection as the whole plan are the ones most often disappointed.
